How do I create an
OnPatient Account?

OnPatient is a patient portal developed by DrChrono EHR to connect providers who use DrChrono as their electronic health record (EHR) with their patients. To create an OnPatient account, you will need to accept the email invite that has been sent to you from our office. The invitation will be sent to the email address you provided at the time of registration. If you do not have the invite, please contact our office at 304.485.3345.

Once you have accepted our invitation, you will need to verify your identity through your date of birth and phone number or social security number. You will also need to create a unique password. Once your identity has been confirmed, you will be able to view your virtual onboarding date to complete you online documents.

• Click the “check in” button.

• Complete all of the necessary information requested.

• All information must be completed in it’s entirety to schedule with a provider.

• PHQ9 / GAD7 (all adults and children over the age of 11).

• Adult Client Onboarding (ages 18 and up)

• Child / Adolescent Onboarding (anyone under the age of 18)

• Review and sign all consent forms.

Once all information is completed, someone from our office will reach out to schedule your first appointment. Please give us a call with any questions.
